Acceptance of Terms

By providing your mobile telephone number and affirmatively opting in to receive SMS messages from Supportive Healthcare and Medical HouseCalls, you agree to these SMS Terms of Service. These Terms govern participation in our text messaging program.


Program Name and Description

Program Name: Supportive Healthcare and Medical HouseCalls SMS Communications Program.

The program may send:

  • Appointment reminders, confirmations, and scheduling updates
  • Patient care coordination and healthcare service updates
  • Administrative and operational notifications
  • Follow-up and customer service communications


Explicit Consent and Opt-In

Participation requires express permission. Users may opt in by completing an unchecked web-form checkbox, signing a patient intake or registration form containing SMS consent language, texting an advertised opt-in keyword, or completing another documented affirmative consent process.

Consent is not implied. Supportive Healthcare and Medical HouseCalls does not use pre-checked boxes for SMS consent. Consent to receive SMS messages is not a condition of purchase or a condition of receiving healthcare services, treatment, or benefits.


Recommended Web Form Consent Language

By checking this box, I agree to receive SMS messages from Supportive Healthcare and Medical HouseCalls regarding appointments, scheduling, care coordination, administrative updates, and service communications. Message Frequency and Charges

Message frequency varies based on appointment activity, care coordination needs, patient requests, and service or administrative updates. Message and data rates may apply. Charges are determined by the user’s wireless carrier and mobile service plan.


Opt-Out Instructions


Users may opt out at any time by replying with any of the following keywords:

  • STOP
  • CANCEL
  • UNSUBSCRIBE
  • QUIT
  • END


After an opt-out request, one final confirmation message will be sent. No additional SMS messages will be sent unless the user subsequently opts back in.


Opting Back In

A user who previously opted out may rejoin by submitting a new affirmative opt-in through an available enrollment method or by contacting Supportive Healthcare and Medical HouseCalls and completing a new documented consent process. Rejoining provides new consent to receive SMS messages.

Privacy and Mobile Information

Personal information is handled in accordance with the Supportive Healthcare and Medical HouseCalls Privacy Policy.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.All categories above exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.


Prohibited Content

Supportive Healthcare and Medical HouseCalls will not knowingly send illegal, fraudulent, misleading, deceptive, harassing, hateful, or otherwise prohibited content, including content promoting unlawful goods or services. Users may not use the SMS program for unlawful purposes.


Carrier and Delivery Disclaimer

Wireless carriers are not liable for delayed or undelivered messages. Message delivery may be affected by carrier operations, network availability, device capabilities, geographic location, or other circumstances outside the control of Supportive Healthcare and Medical HouseCalls. Uninterrupted or error-free delivery is not guaranteed.


Compliance with Industry Standards

Supportive Healthcare and Medical HouseCalls intends to operate the SMS program in accordance with applicable federal and state laws, wireless carrier requirements, A2P messaging requirements, and recognized messaging industry standards. Users must also comply with applicable law when interacting with the program.


Legal Compliance

The SMS program may not be used for illegal activity. Supportive Healthcare and Medical HouseCalls may suspend or terminate messaging when misuse, abuse, fraud, unlawful conduct, or a violation of these Terms is suspected. Nothing in these Terms authorizes conduct that violates applicable federal, state, or local law.


Modifications to These Terms

Supportive Healthcare and Medical HouseCalls may update these Terms. Revised Terms will be posted publicly with an updated effective date. Continued participation after revised Terms are posted constitutes acceptance of the revised Terms, subject to applicable law.


Consistency with Other Materials

Program descriptions, opt-in forms, privacy disclosures, sample messages, and promotional materials should be consistent with these

Terms and the Privacy Policy.


Accessibility

These Terms should be publicly accessible without requiring a login and may be linked in the website footer alongside the Privacy Policy.

By opting in to SMS communications, the user acknowledges having read and agreed to these Terms and the associated Privacy Policy.
